Self Referral

Most referrals to other specialists will require you seeing or speaking with a GP. However, there are a number of local services, listed below, that you are able to make self referrals to.

Sexual Health

You can search all types of sexual health services available (including PEP and reproductive health) at SXT Sexual Health Services. Our closest sexual health clinic is Burrell Street.

For home STI testing, regular and emergency contraception

Southwark residents are able to access free NHS sexual health services online from Sexual Health London.

Physiotherapy

The easiest and quickest way to get NHS physiotherapy help is through the Getubetter app that is run in collaboration with Guys and St Thomas’ physiotherapy service. This allows you to instantly access advice and tailored exercises to help you get better from your musculoskeletal problems. Please use an email address that is unique to you and select the condition you need help to manage. You will then be sent an email with all the information you need to get started.

Feeling low, stressed or anxious ?

You can self-refer for Talking Therapies. This would be the same process your GP would have to follow and your waiting time may be shorter if you start the process yourself. Visit Talking Therapies website to start the process.

Maternity Care at King’s College Hospital

King's College Hospital antenatal self-referral form You can choose to refer yourself directly to the antenatal services at King’s College Hospital, rather than via your GP. Please complete this form, giving as much detail as possible so that we can ensure the best pathway for your maternity care.

Maternity Care at Guy’s and St Thomas ‘

We want everyone to have continuity of carer during their maternity care. This means you are linked to the same team during your pregnancy and postnatal care. This helps build a relationship with your midwife and their team. Research shows this leads to better outcomes for both mums and babies. It can also give you a more positive experience throughout your pregnancy.

Happy Feet

Happy Feet is our not-for-profit toenail cutting service which can be arranged at a local clinic. We provide basic toenail cutting and filing as well as cutting fingernails. Our staff are trained to a standard set by Southwark Foot Health Services in agreement with Age UK Lewisham and Southwark. They cut your toenails, check your feet for any problems and refer on to NHS services where necessary.

Continence

The continence assessment – you can self-refer to a local hospital by calling them between 9am and 5pm (Monday to Friday).

Eye Problems

Eye problems such as redness, sticky eye, watery eye, pain or discomfort, recent onset of flashes can be dealt with by a local optician.

Community Foot Health

Patients must be registered with a GP based in Lambeth or Southwark. We accept referrals from health professionals or self-referrals. We also accept referrals from friends, carers or relatives on the patient's behalf. Referrals will be triaged according to priority and placed on a waiting list. We'll contact the patient, by phone or in writing, inviting them to make their assessment appointment.

Child Health

Is your child under 15 years old and affected by Asthma, Constipation, Eczema or Epilepsy? Then please complete their health check at Take the CYPHP Health Check. It will give your child’s health team a better understanding of their health and wellbeing, which will be added to your child’s GP record. CYPHP will provide you with a copy of the results and a Health Support Pack with practical information on how you can support your child to manage their health condition.

Over 50s

Any other non-medical issues for over 50s – if you need help with any issues affecting safe and independent living please tell us and our local primary care navigators will be in touch – see SAIL check list for all the issues that can be dealt with this way.

The Healthier You: NHS Diabetes Prevention Programme (NHS DPP) identifies those at diabetes high risk and refers them onto a behaviour change programme.
